Job Description
Responsibilities:
- Conduct casework and deliver psychosocial support to patients, caregivers, and families.
- Review intervention plans and collaborate with all relevant stakeholders, including external agencies.
- Manage patient admissions and perform assessments for financial assistance.
- Plan, organise, and oversee social programmes tailored for patients.
- Maintain and update patient records and billing information.
- Lead and facilitate group work and programmes designed for targeted client groups.
Requirements:
- Degree in social work with accreditation.
- Minimum of 2 to 3 years of relevant experience.
- Excellent teamwork abilities, along with effective written communication and interpersonal skills.
