Remote Cardiometabolic RN (Full Time)

We are looking for an RN to deliver patient-centered health coaching and chronic disease management via chat, phone, and video. This role includes triaging symptoms, providing education on diabetes, hypertension, hyperlipidemia, and weight management, coordinating care with providers and cross-functional teams, and documenting patient care accurately in a telehealth setting.

Requirements

• Strong empathy, communication, and patient-centered care skills
• Bachelor of Science in Nursing (BSN) required
• Active Registered Nurse license in good standing, with ability to obtain additional licensure if needed
• Certified Diabetes Care and Education Specialist (CDCES) or willing to pursue if not currently certified
• 3+ years of direct patient care in an ambulatory or telehealth setting
• Adaptable, self-directed, and comfortable working independently
• Proficiency with digital tools for care delivery
• Flexibility to participate in rotational weekend or holiday coverage
• A HIPAA compliant, distraction-free workspace
